MTMCare, Inc., a Hood River-based company providing medication therapy management (MTM) and clinical pharmacy services (CPS) services to high-risk patients experiencing medication-related problems, is seeking motivated individuals for the role of Telephonic Consultant Pharmacist. This position provides medication therapy management (MTM) services from a home office while receiving the support of MTMCare’s Hood River call center. All Telephonic Consultant Pharmacists become part of our nationwide network of Consultant Pharmacists. This is an excellent opportunity to create your own schedule, work independently, provide direct patient care, and improve your clinical skills. We are currently looking for pharmacists who are licensed in AZ and WI, and who are HIV specialists. We are also seeking bilingual pharmacists who are capable of conducting patient interviews in languages other than English.

Primary Responsibilities Include

• Review prescription records and consult with patients in their primary language to improve their health outcomes through optimized pharmaceutical care

• Connect with patients in a caring and compassionate manner

• Provide Medicare-required MTM services, including comprehensive medication reviews (CMR) and targeted medication reviews (TMR.)

• Work with patients to create Medication Action Plans in their language that will help drive outcomes

• Communicate with providers professionally and effectively

• Ensure smooth and timely execution of MTM services
